A lot less than WoW fees...and of course a lot less content...Keep in mind your paying for extra content added to these and to keep world fresh.
You often also pay for quality.
I am not saying Guild wars is not a bad game....but when you take GW and WoW ..sit them on a scale...WoW wins on content side.
WoW is not the only one to charge either

The list can go on....Guild Wars as gkm said is instanced and heavily instanced to sort server load. I think of Guild Wars as a Modern Online Diablo 2.
For discounts for International? They do have WoW USA/WoW Europe and rest of world....However SA is very low on their list to "support" Mostly cause of lower demand. Having 600 WoW Fans is not enough to have a local server support by Blizzard....and of course Shaftkom
I dont mind monthly fees...and good for Blizzard for making a decent MMORPG that has over 5 million users.
Local and Emulated servers are generally terrible...as everyone says what is point playing with 3 people? Half the skills dont work, half the monsters dont work as they should.....broken quests and outdated.
I would rather not play an MMORPG at all than play on an emulated server.

Why do MMORPG's Charge to Play?
All successful MMORPG's evolve, over time. New quests are added, new items, new abilities, even totally new character types. The game world can grow, whole new lands can spring up, new monster type suddenly "invade" the world. The list of possibilities as far as game growth is nearly limitless. But, for this, there is a cost. There needs to be a team on hand to devise these changes, to conceive of the continuing storyline, to draw/render the art, to incorporate it into the coding. And these staff members, if the game were to be updated, say, monthly, can be involved in this game evolution as a fulltime job. Continuing costs to the company equal subscription costs to users, in the end.
Blizzard also stated they had no idea they would even have this amount of players.
