I setup Zoiper using Telkom's IP voice service. The account connects, I can make & receive calls but can't hear the other part, but they can hear me.. I tried toggling the Network settings (TCP/UDP/TLS), enabled/disabled stun server, enabled/disabled encryption etc but can't seem to get it working..

Any ideas on what the issue could be?

Try toggle the SIP ALG setting on your router. Either on or off, whatever is the opposite of what it currently is.

that is strange, I would try another VOIP provider like Fresh Phone just to see if you had the same problem. My guess this is something on Telkom's side, perhaps a codec issue or something. See if you get the same issue with a different provider...

Some suggestions:
  1. Under Audio --> Device - do you have the correct headset/speaker set under Output Device (hey I know, silly suggestion, just mentioning it for the sake of completeness)
  2. If you download Wireshark, do you see the Media (RTP) packets flow in both directions? That should help to troubleshoot if issue is on your computer or upstream
  3. For the port number used for outbound Audio, check your firewall to see if it may being blocked
  4. Sorry I see its giving issues on Android device, also try same account on PC Zoiper and see if it goes away
