Apple states "A normal battery is designed to retain up to 80% of its original capacity at 500 complete charge cycles". 4 months is about 120 days, even if we assume he uses a full cycle every 2nd day (i.e. charges 50% a day), that equates to 60 complete cycles since he bought it. 1% for 60 cycles translates to about 8-9% after 500 cycles, which is less than half Apple's stated amount. If anything, it shows the battery health percentage displayed is not a true reflection.
Also, and to put it bluntly, if your battery is struggling after a couple of years but everything else on the phone is doing well, just buy a replacement battery?100%, agreed!
We also need to take into account that each battery varies in terms of its full charge capacity, even though the spec sheet says 4441mAh, some of us have anything between 4450mAh-4500mAh etc… When it shows 99% health that’s when it has now dropped below that 4441mAh design capacity, so the point at which this happens will vary due to a number of factors around usage and charging habits etc…
People put far too much stock on that Battery Health number, sometimes it sits at 100% for a full year then drops multiple % in the space of a few days/weeks sometimes.
That is the other thing, I remember this was also something that happened on Nokia phones many years ago, it would should full for a very long time then begin dropping faster after the first bar dropped.Apple's battery readings aren't linear. The 100 - 99 drop takes a long time but the 99-95 etc happens much quicker.

Yep indeed and one of those phones is always an iPhone of their choice (a lot of them loved the 12 Mini and 13 Mini or the small Pro).They all carry two phones.
