I ran ping plotter and the 2nd hop which is the isp router is ALWAYS above 10% packet loss, not what it used to be months ago.. and ISP says my line is perfect... so is this normal for an ADSL internet of 4MBits/s speed?
I think 10% is high, but it is probably normal on an ISP with a very high contention ratio. I've also noticed packet loss and Internet slow speeds when the bandwidth to the DSLAM at a Telkom exchange is exhausted.
Get a small capped account for one month from another ISP and compare.

You're probably on a WISP, so 10% could be normal, depends on conditions, how many other people are on the service etc.
Without info we cannot help, also this is a South African tech forum, so we do not know how products operate there.
