Surname changed without consent

Mmmm.... That form might not be the correct one or needed. . If I understand te website correctly, no form is needed and they need to update their system at your request and in writing.
Yes but like I explained they are telling me they can't just change it without a divorce. They keep giving us shitty stories and want money!

My wife experienced the same thing. We were married for a few years already and upon renewal of her passport, she was informed that she had my surname, even though she elected to keep her maiden name. Apparently that's just how the system works.

All it took was for her to complete a one page document to request that her surname be changed back. No costs needed to be incurred. This was around three or four years ago.

And how long before it was changed?

It was pretty quick. It had to be processed before she could get her new passport, and she received hers along with mine so there weren't any delays. Probably a couple of weeks at most?
 
The other problem I am facing now is I am trying to get a loan from standard bank and when it comes to taking fingerprints it won't except them because the fingerprints are in my married surname. I went to the police station and got an affidavit. Hope that helps because they want a hanis verification letter from home affairs, but its going to show the changed surname surely?

Your fingerprints are attached to your ID number.
If the bank suggested that you go to DHA for verification then there's something wrong with the fingerprints attached to the ID number - nothing to do with name. As others have suggested, could be a case of stolen identity, and if so DHA may be aware of it and have flagged the ID for investigation.

Only DHA can advise on the reason as to why your prints failed at the bank if the bank suggested you need to go to DHA for verification. So start there by getting your prints verified, and see what the document that gets produced by Home Affairs has to say on it.
 
Be very patient and friendy. Take a few days leave and try a few different home afairs until you get someone not looking for a bribe. Or negotiate the bribe down to R500 or something more affordable for you.

Then also start with the simple things like just replacing any documents destroyed in your house fire. Doesn't matter if it's wrong and go from there.

Or....take a picture of the person and post it on twitter while standing there with HA and who ever is now the head of HA and tell them the branch wants a bribe to do their job. See what happens.

It shows the married surname.
 
What about changing your name via deed poll? Its nothing like R9000

Fellow from Sri-Lanka, who had a 24 letter surname which no-one could get right, had it changed to Chilaw, where he was born

Sure it can be done but you seem far to preoccupied with the name in your ID when you should be worried about the very real probability that someone is walking around with your ID number in their ID book.

The name was changed without my knowledge. There is nobody walking around with my id I have already checked on this.
