Thanks pretty insightful, it was bascially one of the following:

1. Structural Design was incorrect (Engineer in trouble)
2. Main Contractor Build incorecctly, Didnt use correct rebar etc. (Main contratcor in trouble)
3. Loading during construction / methodology incorrect (Main contratcor in trouble)
4.. Materials incorrect (Concrete didn't reach correct strength or rebar might have been sub-par etc. (Supplier and Main Contractor in trouble)

Either way I hope they release the full report and responsible party needs to be held accountable.
 
One some level
I understand why people are not named these days. People go feral. Doxing is real.
People who are not responsible get dragged and they go in hiding making them look guilty.
And we as the public is so gatvol we do the wrong things.

But on the other hand we can find a crook in two ticks

The engineer is responsible for the design of any engineering aspects of theproject that require rational assessment or design. Typically these are aspects that are not covered by the “deemed-to-satisfy” provisions of the Building Regulations. These could include investigation of site conditions, selection ofappropriate types of construction and the specification of materials, design ofstructural elements (foundations, beams, suspended slabs, high boundary and/orretaining walls, etc.) or services (ventilation, water, electrical, mechanical, fire andother installations) . The engineer should inspect the work for which he assumesresponsibility and certify that the design has been correctly implemented.

Unless specifically requested to do so, the engineer does not supervise the contractor or take responsibility for the contractor’s workmanship.
