Damn, i did the watt bike test and I only scored level 4 (out of 5) for cardio and that is with me doing 2 workouts a week where i average above 70% of max HR for 40 minutes, so i consider myself fairly fit. It may be that the treadmill test is the way to go for full points.
The Biokineticist that I went to said the treadmill test is 6 minutes long. The watt bike test can go on indefinitely, in theory. The aim there is to get your heart rate to 85% of max HR, and for me this was pushing 115W on the bike. You start at a target of 55W for 45 seconds (or 1 minute I forget now) and then every interval it bumps up by 10W if i remember correctly, so I suppose I did 7 minutes which was killer on my legs more than my lungs.
I didn't do any 'lie down and in fetus position' assessment. I also had to do one legged half squats x3 each leg, normal overhead squats, doing 3 lunges and 3 pushups whilst keeping my back and neck straight etc...
