I have the same issue with my mothers iPhone. What makes it more complicated is that I live in Australia and she is in Fish Hoek, so we cannot easily check her phone or talk to Vodacom. It uses small amounts of her pre-paid data every hour all day long, 24/7, so using up around R2.40 per day or R 72 a month. See a screen shot of her bill below. She has wifi in her appartment so it makes no sense that the phone uses the pre-paid data instead of the wifi. She is a pensioner and hardly ever leaves her appartment so it is not while she is out of range from her wifi.
We have turned off everything she does not need. We only leave her mobile data switched on in case her wifi goes down and we need to get hold of her.
Any help to resolve this will be appreciated.
