Only lost packet here is on your LAN (first hop) which carries through the entire chain. Are you connecting via LAN cable or WiFi? Rest of the tests don't show any obvious packet loss.
1 Lost packet at hop 17/18 (deep into the AWS network) - maybe allow the test to run longer (+-3-5 mins) to see if this is consistent.

Will try and get another server IP. There is around 3 as far as i can remember. Pings to London was 142ms but in game was around 165ms - 180ms

The second MTR posted also showed a first hop loss (a single packet) - however, this time it wasn't commutative (1 lost packet at an early hop didn't translate into a lost packet later on).
We hand over to Amazon (AWS) in Cape Town directly and there are no losses up until the handover or even onwards through their transit to this server, albeit the last few hops are showing weird results.
I've been running an MTR to this IP all morning and I'm seeing the server not replying to all ICMP requests (the loss ratio between hop 18 and the server itself is disproportionate). This points to the server either having issues or ICMP being reprioritised which makes ICMP a pretty useless tool for diagnosis on this server.

Haven't seen packet loss in any of your results so far. Running MTRs to all the above addresses as well as reverse from VPSs hosted on AWS in EU Ireland. Will let you know if I see anything.
I’ve picked up that AWS is sending us all traffic via JHB. If you dropped a few packets in the past few mins, it’s because we tried to force all inbound AWS traffic to our CPT peering interface which did not go as planned.
We’ve escalated to AWS network ops for feedback on what they’re seeing from their end.. This will help drop Cape Town latency to +- 150ms from EU.
