Showing 0 loss here... remember, it's only loss if it carries through to the last hop. Hop 2 is Vumatel's core in KZN, and it doesn't reply to every ICMP request (It's still pushing packets).
OK so the first one with 6% loss on hop 4 doesn't affect anything?
Nope. The way MTRs work is by analysing the path of packet and then establishing individual ping sessions (ICMP requests) to each of the IPs along the path. However, some devices don't always respond to ICMP requests (eg, replies are rate limited to minimise strain on these devices). MTR is a tool that gives us an idea of where losses may be occurring if the loss carries through to the end. But if the last hop has no loss, then packets are getting to their destination just fine.
That said, still investigating what could be wrong here - TCP issues like an MTU mismatch could cause the above issue described by @JOEY_8.
Durban update - we are aware of an issue currently affecting users in KZN. We are investigating this

It looks like one of our upstreams just introduced additional routes via PCCW. There's a little route leaking going on because we have common peers (IS). Clearing that quickly while we wait for some feedback.

Yeah, this is what MTR says, however in my game it is 400 ping with lots of ping packet losses.

Issue identified and sorted out.
We replaced one of our core switches in Durban today, however, the replacement unit we had on hand in the area seems to be creating more issues than good for KZN clients. We're shipping down another unit from JHB first thing and will arrange for a replacement during the course of tomorrow. We do apologise for the inconvenience caused by this device. We're moving as much traffic off it as we can for now.
