I think the major issue here is that complaints aren't public - they're going to CISP, who are banging their heads against Octotel's wall. Perhaps if everyone having issues in this area starts making a fuss on a public forum, like their FB page or Twitter, we may get some attention?
It is run by marketing people. They have no clue...

Indeed- but I did escalate an issue through them and they got it to the right people. Marketing people would report a whole lot of negative feedback.

I don't currently have any of the issues you seem to be complaining about (even though my iperf shows massive UDP packet loss to JHB and a little to EU, I don't see any of it reflected in my download speed or latencies in-game), so either I'm missing the issues or it's your line that's the issue in this case - we can easily check that by running the same tests on your line and my line?

Ping me if you want me to duplicate any tests you do on my line so we can compare if it's an area problem or a line problem.

Over TCP, things look fine locally and internationally, it looks "okay" but not nearly as good as on UDP - although definitely not the 4mbit you're getting - see below.

Swapped to ubuntu to do these tests btw, so we can take the tcp window scaling out of the equation :)

Over TCP, things look fine locally and internationally, it looks "okay" but not nearly as good as on UDP - although definitely not the 4mbit you're getting - see below.

Swapped to ubuntu to do these tests btw, so we can take the tcp window scaling out of the equation :)

I thought it was only a problem with Windows with uploads? My upload test is identical to the download one above. Current Speedtest multi to Coreix is around 45mbit download.
