As far as I know the people to speak to is 0821940 (business support) as they handle the wimax product. Have you tried them?

What type of service are you looking at OMG!!! assured or just normal broadband ?

Once you meet all the requirements as mentioned there then give them a call.
The service is extremely dependant on coverage (if you get 3G/HSDPA signal in your particular area it will NOT be indicative of Wimax coverage in your area)
The tariffs between Broadband (similar to ADSL offerings) & Assured Rate (think of a normal Telkom Leased Line to an ISP with an internet access loaded on top of it and a guarantee if speeds)
