Boot into safe mode and search for the file in the registry, remove all entries, ensure that you kill all suspect processes before doing so, run AV and reboot. Might work.

No idea, never used norton in my life but a quick google reveals this (http://www.google.co.za/search?client=firefox-a&rls=org.mozilla%3Aen-US%3Aofficial_s&hl=en&q=ccapps.exe&meta=&btnG=Google+Search)

and you are using windows xp, switch off the system restore function. some of the nasties copy themselves there, to be restored on the next reboot.;)
