We have launched our latest broadband survey, giving you the chance to rate your broadband provider and ADSL ISP, and also tell them what you would like to see from them.

This survey will take less than 10 minutes to complete, and you will stand a chance to win a brand new iPad 2 for completing the survey.

This is a fail. You're asking to rate various different providers, yet you're providing the same survey for each one.
So how can I rate each provider when you can only select one answer on the single survey?
 
If we had one of the cell operators as a broadband provider, but cancelled their service recently can we still do the survey?
