It all depends on the waiters service
Normally 10%/15% but on cheap bills much more.
If the service is very bad and not just a mistake that anyone can make then the amount reduces
If it's the kitchen at fault and not the waiters fault, I would point it out but not penalize the waiter
 
Usually 20% but with alteration downwards depending on service. Once in a pretty nice, upmarket restaurant, noticeably tipped zero because the waitress was snooty northern suburbs, slow, barely looked at anyone when taking and order and got a side order wrong and was uptight when asked to please correct it.
 
A base rate of 10% with a minimum flat amount of R20.00-R30.00.

Poor service can result in no tip, truly excellent service, especially when a large group/table and they were kept quite busy, I go higher, 15%-20%.

I also make a habit of not only talking to a manager if service is bad, but make a point of praising excellent service. If you are not willing to praise excellent service, I do not believe you have the right/licence to complain about bad service.
